Adding Athena Chart Context to Your Notes
You can add information from a patient's Athena chart to help Attend generate more complete, context-aware documentation.
Chart context is optional. Select only the information that is relevant to the current visit.
What context is available?
Health profile
The health profile can include:
Patient demographics
Active medications
Active problems
Active allergies
Social history
Surgical history
Family history
Gynecologic history, when available
Pregnancy history, when available
Perinatal history, when available
Availability depends on what is recorded in Athena and which department is associated with the visit.
Visit notes
Attend can display recent Athena encounter documents. Each item may show information such as:
Document type or title
Document date
Provider
Document status
Select the prior notes that are relevant to the current encounter rather than attaching every available note.
Reports
Attend can display recent lab and imaging documents from Athena.
Some Athena documents contain text that Attend can use directly. If a document is available only as a PDF or does not expose readable text through Athena, you may need to review it directly in Athena instead.
How to add Athena context
Start a new encounter in Attend.
Choose the option to add content or context to your note.
Select Add from EHR.
Select the correct patient and appointment.
Review the available health profile, visit notes, and reports.
Select the items that are relevant to the current visit.
Continue with the encounter and generate your note.
Context is a snapshot
Athena context reflects the chart at the time it is added to Attend.
If the chart changes afterward—for example, if a medication is updated or a new lab result arrives—the context already attached to the Attend encounter does not automatically refresh. Add the updated context again when the newer information is important to the note.
Best practices
Verify that you selected the correct patient before attaching chart information.
Prefer the health profile for a broad clinical summary.
Add only the prior notes and reports that are relevant to the current encounter.
Review the generated note for accuracy even when chart context was included.
Confirm time-sensitive information, such as medications and results, directly in Athena when needed.
What adding context does not do
Adding Athena context does not:
Change the patient's chart in Athena
Automatically update the patient's problem list, medication list, or allergy list
Keep the Attend context synchronized with later Athena changes
Send the generated note back to Athena
To return the completed note to the chart, review it and use Send to EHR after the encounter.
