The format used depends on which Elation structure your Attend template has been mapped to.
Default SOAP Mapping
When using Attend's default SOAP note template, the integration uses Elation's SOAP template format.
Standard SOAP sections map as follows:
Subjective → Elation's Subjective section
Objective → Elation's Objective section
Assessment and Plan→ Elation's Plan Section.
Custom Template Section Mapping
For custom templates created for your organization, the Attend team configures unique section mappings based on your clinical needs.
How Custom Mapping Works
The Attend team works with your clinical leadership to:
Understand your documentation requirements
Review your custom Attend template sections
Identify the appropriate Elation note format to use
Map each Attend section to the corresponding Elation field
Examples of what can be mapped:
Chief Complaint → Elation's Chief Complaint field
History of Present Illness → Elation's HPI field
Past Medical History → Elation's Past Medical History field
Review of Systems → Elation's ROS field
Physical Examination → Elation's Exam field
Assessment and Plan → Elation's Assessment/Plan fields
And many other fields depending on your template structure
⚠️ Important: Each custom template has its own unique mapping configuration. Sections from one template may be mapped differently than similar sections in another template, based on clinical workflow needs.
Unmapped Templates and Simple Notes
If you're using a custom template that has not yet been mapped to specific Elation fields, Attend will use the Simple Note format in Elation.
How Simple Notes Work
With Simple Note format:
All sections from your Attend note are combined
The complete content is placed in the "narrative" field of the Elation Simple Note
Section headers from Attend are preserved in the narrative
The note maintains its structure, but isn't divided into discrete Elation fields
When this happens:
Your note still arrives in Elation
All content is preserved
It's just formatted in one large text section

Viewing Your Template's Elation Format
Currently, you cannot view which Elation note format or field mappings your templates use from within the Attend interface.
To find out how your template is mapped:
Send a test note using the template
View the resulting note in Elation
Observe which Elation note format is used and where your content appears
Alternatively, ask your clinical team leader for documentation about your template mappings.
Requesting Template Mapping Changes
If you have feedback about how your templates map to Elation, or if you'd like to request changes to the mapping:
Step 1: Identify What Needs to Change
Be specific about:
Which Attend template you're using
What's not working in the current mapping
Where you'd like content to appear in Elation
Which Elation note format you'd prefer (if changing)
